This course provides an introduction to the purpose & objectives of Marine incident investigations. Every accident or incident is a learning opportunity, but one that could be wasted unless effort put into analyzing it focuses on discovering the true underlying causes of the incident rather than focusing on the people directly involved & the immediate causes of their failure.
If properly investigated, each incident is of high learning value & it is very important that the lessons learnt are put into full use so as to avoid the re-occurrence of the same types of incidents. The only way one can learn the proper lessons is by learning the structured methods of root cause analyses in order to find the true root causes of such incidents. Only then can the correct preventive actions be initiated & implemented.
